Starter Kit Inclusions

The Starter Kit includes:

 

• A laminated QR code to provide to patients.

• A framed 6x4 inch QR code for display in the waiting room.

 

Reminder: Patients register for BnTRx using the QR code.
 

Also included:

• HPS Pharmacy New Account and Delivery Preference Form. Reminder: This form is also available in the Resources section of www.bntrx.com.au. It must be completed, scanned, and emailed to b.toxin@hpspharmacies.com.au before starting.

• Co-payment Authorisation and Consent Form. Reminder: Also available on the Resources page. This form is for patients who are unable to register via the QR code on their mobile device. After entering the details into BnTRx, retain a copy of the form for your records.

• A contact card with HPS Pharmacies’ details.
